I am looking for a form that opens up on a particular page for a while only. Like the one that comes on this website. http://www.ypigroup.com/ The for charter one.

It need not necessarily be in the menu bar. It can be on the right as well, but it should open up when the homepage is opened only, and not on every page..

You can easily do this task with this plugin https://wordpress.org/plugins/popup-contact-form/ , you just need to install this plugin and then add this plugin on your Home page then whenever someone want to contact you he/she can easily contact you from home page.

Enable this contact form only on home page then this contact form never appear in other pages...
